Under a general anaesthetic, local anaesthetic is infiltrated into the skin of the upper eyelid, and the redundant skin is removed with any bulging fat repositioned or removed. Following this, a small sleeve of the eyelid muscle is usually plicated, and the incisions are sutured using extremely fine dissolving sutures. This work reflects sustained clinical experience with suture-based eyelid techniques rather than a single procedure or short-term outcome. Over time, genetics, skin quality, facial structure, and natural tissue descent can affect how the eyelids rest and move. Crow’s feet form in the outer eye region and are not corrected by upper blepharoplasty alone. They may be improved with complementary treatments such as skin resurfacing or other non-surgical options. Less common complications may include delayed  wound healing, visible scarring, infection, uneven crease formation, incomplete eyelid closure during early healing, or the need for revision surgery. Pictures are shown at 2 weeks post op with slightly more swelling remaining on the left compared with the right eye. Results show a completely transformed eyelid appearance with loss of the hollow sinking caused by incorrect attachment of the upper eyelid levator. This patient in her 20s had a subtle natural double eyelid crease in her left eyelid, however not fully formed and no crease in her right eyelid and sought help with correction of her asymmetry.
A lower lid blepharoplasty is a much more difficult operation and is less predictable than the upper lid blepharoplasty. The incision is placed just under the lid margin below the eyelashes.Once again the lower lids cannot be observed in isolation and need to be assessed with the midface (cheek region). The same risk of blindness exists as with the upper lid however the main risk from the operation is called ectropion or in its mildest form – scleral show. Typically the recovery involves bruising and swelling (similar to a black eye) for about a week.

Keep in mind that it can take several weeks for the swelling to fully subside. One of the most common problems is excess or descending skin around the eyelids, which can rest on the eyelashes. Temporary side effects may include swelling and bruising during the post-operative recovery. Medicare may provide rebates for upper eyelid surgery if excess skin significantly obstructs your vision. You’ll need photographic evidence and documentation from your surgeon showing the medical necessity.

However, if excess skin and fat around your eyelids are causing visual impairment or other significant functional issues, the procedure may be deemed medically necessary and qualify for partial coverage by Medicare.
